Semaglutide is a prescription medication. Semaglutide is FDA-approved and clinically proven to help with weight loss. How? it acts like a natural hormone your body uses to regulate blood sugar and appetite-key steps in resetting your biological set point to help you maintain a new lower weight. In fact, in the pivotal clinical trial people who combined Semaglutide with lifestyle changes lost an average of 9% of their body weight after 20 weeks, and 14.9% after 68 weeks
HOW DOES IT WORK?
Semaglutide works by mimicking the hormone GLP-1, which slows digestion and helps your brain regulate appetite by reducing hunger sensations. As your digestion slows, you may feel full longer and therefore eat less, which can lead to weight loss
Semaglutide dosing starts out low, then gradually increases over the first few months until a steady dose is reached. If prescribed, Dr. Eberle's team will work with you to find the schedule and dosing that's right for you. As an example, Dr Eberle may start you out on the lowest dose, 0.25 mg injected once weekly for the first month and slowly ramp you up to the maximum dose of 2.4 mg once weekly over the course of several months. This will depend upon your specific situation and your tolerance of any known side effects.
Dosing-Semaglutide and 8-12 Injections (Cyanocobalamin) Smg: Dosing 0.2ml, once a week, for the first 4-5 weeks; then Dosing 0.4ml, once a week, for the remaining weeks.
HOW DO YOU TAKE SEMIGLUITIDE INJECTIONS?

IMPORTANT SAFETY INFORMATION
It is not known if Semaglutde can be used in people who have had pancreatitis.
Semaglutde is not for use in people with type 1 diabetes.
It is not known if Semaglutde is safe and effective for use in children under 18 years of age.
THE MOST COMMON SIDE EFFECTS OF SEMAGLUTIDE MAY INCLUDE:
- Nausea
- Diarrhea
- Stomach (abdominal) pain
- Vomiting
- Constipation
IF YOU EXPERIENCE NAUSEA, HERE ARE SOME GENERAL NAUSEA TIPS THAT YOU MIGHT FIND HELPFUL:
- Eat bland, low-fat foods, like crackers, toast, and rice
- Eat foods that contain water, like soups and gelatin
- Avoid fried, greasy, or sweet foods
- Avoid lying down after you eat
- Go outdoors for fresh air
- Eat more slowly
- Drink clear or ice-cold drinks
DO NOT USE SEMAGLUTIDE IF:
- You or any of your family have ever had MTC or if you have MEN 2.
- You are allergic to Semaglutide or any of the ingredients in Semaglutide.
- Before using Semaglutide, tell your healthcare provider if you have any other medical conditions,
including if you:
- have or have had problems with your pancreas or kidneys.
- have a history of diabetic retinopathy.
- are pregnant or breastfeeding or plan to become pregnant or breastfeed. It is not known if Semaglutide will harm your unborn baby or pass into your breast milk. You should stop using Semaglutide 3-4 months before you plan to become pregnant.
Tell your healthcare provider about all the medicines you take, including prescription and over-the-counter
medicines, vitamins, herbal supplements, and other medicines to treat diabetes, including insulin or
sulfonylureas.
Please complete the attached lab requisition at any Quest, Labcorp, or through your PCP. Once we receive your labs and the doctor has approved, we can then start your weight loss plan.
WHAT ARE THE POSSIBLE SIDE EFFECTS OF SEMAGLUTIDE?
Semaglutide may cause serious side effects, including:- Inflammation of your pancreas (pancreatitis). Stop using Semaglutide and call your health care provider right away if you have severe pain in your stomach area (abdomen) that will not go away, with or without vomiting. You may feel the pain from your abdomen to your back.
- Changes in vision. Tell your health care provider if you have changes in vision during treatment with
Semaglutide. - Low blood sugar (hypoglycemia). Your risk for getting low blood sugar may be higher if you use
- Semaglutide with another medicine that can cause low blood sugar, such as sulfonylurea or insulin. Signs and symptoms of low blood sugar may include: dizziness or Light headedness, blurred vision, anxiety, irritability or mood changes, sweating, slurred speech, hunger, confusion or drowsiness, shakiness, weakness, headache, fast heartbeat, and feeling jittery.
- Kidney problems (kidney failure). In people who have kidney problems, diarrhea, nausea, and vomiting may cause a loss of fluids (dehydration), which may cause kidney problems to get worse. It is important for you to drink fluids to help reduce your chance of dehydration.
- Serious allergic reactions. Stop using Semaglutide and get medical help right away if you have any symptoms of a serious allergic reaction, including swelling of your face, lips, tongue, or throat; problems breathing or swallowing; severe rash or itching; fainting or feeling dizzy; or very rapid heartbeat.
- Gallbladder problems. Gallbladder problems have happened in some people who take Semaglutide. Tell your healthcare provider right away if you get symptoms which may include pain in your upper stomach (abdomen), fever, yellowing of the skin or eyes (jaundice), or clay-colored stools.
- The most common side effects of Semaglutide may include nausea, vomiting, diarrhea, stomach (abdominal) pain, and constipation.

- 1st injection administered at Weston Center for Plastic Surgery. One of our registered nurses will properly show you how to administer the injection and will provide you with supplies to continue your weekly injection on your own.
Pictures, measurements, weight, and height will be monitored monthly.
Weeks 2 thru 8
- You will be administering your injection in the comfort of your home/work.
Week 4 or 5
- Schedule your check in appointment, so we can take pictures, measurements, weight, and answer any questions or concerns you may have.
At this appointment, we will also adjust your Semaglutide dosage
Week 8
- Schedule your check-in appointment, so we can take pictures, measurements, weight, and answer any questions or concerns you may have.

TIRZEPATIDE
What is Tirzepatide?
Tirzepatide is a once-weekly injectable dual glucagon-like peptide 1 (GLP-1) receptor and glucose-dependent insulinotropic polypeptide (GIP) receptor agonist. It works by increasing insulin production and lowering glucagon secretion as well as targeting areas in the brain that regulate appetite and food intake. A small change in the molecule allows the drug to last weeks in our bodies rather than the natural version our bodies make, which lasts minutes
How Does Tirzenatide Work?
Tirzepatide acts in the following ways:
- Delays how quickly our stomachs digest food. Leading to a feeling of fullness and satisfaction with smaller meal sizes.
- Slows intestinal motility so you will feel fuller longer after meals.
- Dual action receptor allows for lower blood sugars, in part by reducing the production of sugar in the liver.
- Stimulates insulin secretion by the pancreas
- GIP increases energy expenditure, resulting in weight loss reductions
DOSING
Tirzepatide/B12 Dose: 10mg/500mcg/mi-2 ml vial (20 mg Tirzepatide per vial)
Sample Dose Escalation Schedule*
RX #1: Tirzepatide/B12 10mg/500mcg/ml – Inject 25 units subcutaneously weekly for 4 weeks, then increase to 50 units weekly for 2 weeks (1 x 2ml vial)RX #2: Tirzepatide/B12 10mg/500mcg/ml – Inject 50 units subcutaneously weekly for 2 weeks, then increase to 75 units weekly for 4 weeks (2 x 2ml vials)
RX #3: Tirzepatide/B12 10mg/500mcg/ml – Inject 100 units subcutaneously weekly for 4 weeks (2 x 2ml vials)
RX #4: Tirzepatide/B12 10mg/500mcg/ml – Inject 125 units subcutaneously weekly for 4 weeks, then increase to 150 units weekly thereafter (3 x 2ml vials)
RX #5: Tirzepatide/B12 10mg/500mcg/ml – Inject 150 units subcutaneously weekly for 4 weeks (3 x 2mlvials)

Side Effects/Contraindications
The most common side effects include nausea and constipation. * Patients also report acid reflux, stomach pain, vomiting, and diarrhea. The risk of serious side effects increases in patients with hypoglycemia, kidney problems, and risk of allergic reactions.
*500mcg/mL of Cyanocobalamin added to reduce nausea side effects.
**This medication is not insulin and should not be used if you have type 1 diabetes or if you develop diabetic ketoacidosis. Tirzepatide should not be used with any other GLP-1 medications.
RISK OF THYROID C−CELL TUMORS. If you or any family members have been diagnosed with Multiple Endocrine Neoplasia Syndrome Type 2 or Medullary thyroid cancer you should not take Tirzepatide. If you have ever had pancreatitis consult with your medical provider as using Tirzepatide can increase the risk of developing pancreatitis.
*This is one sample of dose titration. Strive Pharmacy suggests using our variable dose vials to achieve the lowest effective dose for the patient even if it varies from the dosing schedule.
